The following items were collected from various police departments and district courts:

Adams Township

July 11 — Mark G. Cadamore, 55, of Mars was charged with two counts of DUI and two summary traffic violations following a June 16 two-vehicle crash on Valencia Road at Fetzer Road. Police said Cadamore was unsteady on his feet, spoke with slurred speech and smelled of alcohol. His blood alcohol content was 0.325%. A person with a blood alcohol content of 0.08% or more is considered too drunk to drive under state law.

State police, Butler

July 5 — Arnold L. Weitzel, 69, of Butler, was flown to UPMC Presbyterian with suspected serious injuries after the 2004 Harley-Davidson Fatboy he was riding traveled off Eau Claire Road in Venango Township and went into a ditch. Weitzel, who was wearing a helmet and safety glasses, was thrown from the motorcycle and landed about 10 feet away, police said. The Eau Claire Volunteer Fire Company and auxiliary also responded to the accident.

July 7 — Benjamin J. Hostetler, 30, of Harrisville, and Richard J. Ballard Jr., 52, of Slippery Rock, were involved in a two-vehicle crash on Franklin Road in Slippery Rock Township, in which Ballard and his passenger, Charlene L. Janke, 39, were taken to AHN Grove City for injury evaluation.

June 24 — Police investigated a crash on Prospect Road in Franklin Township. Police said Edward G. Clokey, 77, of Pittsburgh, was driving when he drifted off the right side of the road and struck a boulder and a tree. He was flown to Allegheny General Hospital.

July 6 — Police investigated a single-vehicle crash involving an 18-year-old unidentified woman. Police said the driver fled a traffic stop in Summit Township and intentionally crashed into a tree in Franklin Township. She was charged with fleeing and eluding police.

July 9 — Police investigated an incident between a passenger vehicle and a dump truck in Muddy Creek Township. Police said the dump truck was traveling south on Route 19 and failed to secure its gravel load, causing debris to fall onto a Mitsubishi Eclipse belonging to Janene M. Grossetti, 52, of Slippery Rock. The debris caused damage to the Mitsubishi’s hood and windshield. The dump truck fled the scene.

July 9 — Police investigated a report of a fight between siblings in Connoquenessing Township. Police said 14- and 15-year-old boys were charged with harassment.

July 10 — Police responded to an incident in Muddy Creek Township and found Lynzie Johnson, 29, of Ford City, violated a protection from abuse order.

State police, Franklin

July 7 — Leland J. McCalmont, 85, of Harrisville, and Stephen R. Chikosky, 77, of Athens, Ohio, suffered serious injuries in a two-car collision on Interstate 80 in Irwin Township, Venango County. Both men were taken to AHN Grove City by Superior Ambulance.
